BENEFITS SUMMARY
BÂRRX Medical provides eligible employees and family members with a rich and diverse program of health and welfare plans
Paid Time Off (PTO) and Holiday
- Eleven paid company holidays per year
- Three weeks (15 days) paid time off per year for new employees, accrued 10 hours monthly
Medical
- Group coverage either through either a HMO or PPO
- Employee may cover their spouse and or eligible dependents
- Flat rate breakdown for employee, spouse and dependents
Dental
- Group coverage through Delta Dental
- Flat rate breakdown for employee, spouse and dependents
Vision
- Group coverage through Vision Service Plan
- Flat rate breakdown for employee, spouse and dependents
401(k)
- Employee may contribute 1-90% of gross salary on a pre-tax basis through Fidelity e401 (k) up to $15,500 annually, if you are 50 years of age or over, you are elegible to contribute an additional $5,000 during the current year
- Direct Deposit
- Employee may set up for up to two accounts
- Pre-note one pay period before direct deposit is active
- Required: voided check for checking account / an ABA routing # and account number for savings account (BANK PRINTOUT REQUIRED). If more than one checking account, separate checks are needed
Life Insurance
- An amount equal to two times the salary, up to $500,000
Long Term Disability
- Benefit is equivalent to 66.7% before-tax monthly earnings, not to exceed the plan's maximum monthly benefit amount, less other income sources
Flex Spending
- The benefit allows for deduction of dollars from paycheck to be placed into a special account pre-taxed. The dollars put aside can then be used for eligible medical and dependant expenses incurred by the employee. The medical care expenses eligible for this plan are those that are not covered or reimbursed by health, dental or vision insurance
